Students requesting support from SOS are required to provide supporting documentation for every extenuating circumstance except for bereavement.

Instructions for submitting:

When you submit a request to our office, you have the option to include supporting documentation. Below you will find requirements for this documentation, which depend on the type of extenuating circumstance you have experienced. If you do not provide this documentation, we will contact you to request it after you submit your request.

Please review the following list of requirements for supporting documentation carefully.

 

Please provide a letter from a licensed clinician that includes the following:

  • Must be on official letterhead with the clinician’s name, licensure information, location, and contact information

  • Dated within two weeks of your request to SOS

  • States your medical diagnosis and that you are a current patient

  • Describes the current severity of the condition(s) and impact on student's academic and overall functioning, including why standard campus accommodations are insufficient in providing adequate support

  • Type of Request with anticipated start and end dates for the request period
  • Provides professional recommendations regarding treatment or care for the management of this student's condition(s), with a focus on what treatment, support, or specific campus accommodations will help the student to transition back to enrolled student status
  • Signed by the clinician

Please provide discharge paperwork that includes the following:

  • Hospital information (name and location)

  • Your name

  • Dates showing length of hospitalization (last date must be within three weeks of your request to SOS)

Supporting documentation for Medical Withdrawals requires additional information. We request that students schedule a meeting with a team member to learn more about documentation requirements and the withdrawal process. You can reach us at success@mica.edu.

  • Name of reporting officer

  • Location

  • Date of event (must be within three weeks of your request to SOS)

  • Your name

  • Description of crime
