Please submit your request as soon as possible after your concern arises. The sooner we connect with you, the easier it will be to address your concern and make sure you have access to the resources that you need. We can provide faculty outreach up to two weeks after the extenuating circumstance occurs. Medical withdrawals can be processed up to one month after the end of the semester.
If you are unsure which of our support services to request, do not worry. Include as much information in the description section as you can and we will do our best to connect you with the appropriate resource.
When submitting your form, you have the option to include supporting documentation. Please note that supporting documentation is required for faculty advocacy for health events and hospitalizations, being the victim of a crime, and for medical withdrawals. After you submit the student referral form, you can expect to receive a response from someone in our office within 2 - 3 business days. We may request that you submit supporting documentation in order to process your request, in which case, you will receive instructions for submitting. We highly recommend that you communicate with your faculty member(s) about your concerns if you are comfortable doing so. They may be able to offer assistance and help you identify solutions. If you are working with our office to request absence excusal or deadline extension, the decision to grant these requests or not is ultimately at the discretion of your faculty member.
