All required completed forms and scanned documents must be submitted and uploaded through the secure, HIPPA compliant, Medicat portal. Due to medical privacy concerns, we cannot accept any medical forms, documents or immunization records via email.
To access the Medicat portal, please visit the MICA Student Health Center website at https://www.mica.edu/officesdivisions/student-health-center/ and click on the tab for the Medicat Student Health Portal. The login is your username from your MICA email address (without the @mica.edu), and your MICA email account password.
