Forms Needed to Process Your VA Education Benefits

Transfer Veteran Students Applying for Benefits

Any student who previously received VA education benefits at another school (except for students applying for/or using the Survivors and Dependents Educational Assistance) must complete a Request for Change of Program or Place of Training Form (VA Form DD 22-1995). Students applying for/or using the Survivors and Dependents Education Assistance program must submit the Dependents' Request for Change of Place of Training (VA Form 22-5495).

If you have used VA Benefits at another institution as an Active and Selected Reservist:

  • For Chapters 30, 1606, or 1607, complete the Request for Change of Program or Place of Training Form (VA Form DD 22-1995) to declare a new school and new program and to notify the VA of your change.
  • Submit copies of the VA Form DD-214 or NOBE (Notice of Basic Eligibility) and the MICA Veterans Request for Certification Form to the MICA VA Certifying Official.
  • It is important that you submit all official transcripts to the Academic Advising Department for evaluation of prior credits or training.

If you have used VA benefits at another institution as a Survivor and Dependent of a Veteran:

  • Chapter 35, complete the Request for Change of Program or Place of Training for Survivors' and Dependents' Education Assistance Form (VA Form 22-5495) to declare a new school and new program and to notify the VA of your change.
  • Submit copies of the VA Form DD-214 or NOBE (Notice of Basic Eligibility) and the MICA Veterans Request for Certification Form to the MICA VA Certifying Official.
  • It is important that you submit all official transcripts to the Academic Advising Department for evaluation of prior credits and or training.
