MICA Corporate Cards FAQs

Procedure for Fraudulent or Other Disputed Charges on MICA M&T Visa Cards

1. Cardholder must call the number on the back of the card to initiate the dispute.

2. If fraudulent, a new card will be issued and sent by mail (6-7 business days) to the MICA post office. Overnight service may be requested if the card is urgently needed.

3. You must report the charge as a normal charge since MICA auto-pays the charge.

4. If an adjustment or credit is issued, report in Workday using the same code as in step 3.

5. The cardholder is responsible for resolving the issue.

6. Workday does not allow submission of a report without attaching a receipt if a transaction is over $15.00. In the case of a suspected fraudulent transaction, use a missing receipt form with a comment in its description section “Reported to M&T as a suspected fraud charge on mm/dd/yyyy”.

Procedure for Reporting and Reimbursing MICA for Accidental Personal Expenses Charged to Corporate Cards

1. Report all corporate and personal transactions in Workday using normal worktags.

2. Write a personal check payable to MICA for the total amount of personal items.

3. Write the worktags of the original transactions on the check.

4. Fill in a MICA Deposit Form:

a. Fill in the Departmental Deposit section

b. Write each transaction description (“personal use”), amount, and worktags

5. Send via postal mail the check and deposit form to:

Use of Missing Receipt Form

We do not recommend using a missing receipt form. This form should only be used on an exceptional basis, such as for a fraudulent transaction (see #6 above).

1. On the form, you need to enter the date, amount, short description, purpose of purchase and reason for the missing receipt. Do not fill in the transaction id space if it is not available.

2. The form must have your signature and your supervisor’s signature (if there is any missing signature, the report will be sent back).

Procedure for Activating MICA M&T Visa Cards

Activation instructions can be found here.

1. To activate your new card, call the number included with it. There should be a sticker on the front.

2. When you call the activation number, you will be asked to verify 2 pieces of information about your identity, your Social Security Number and your Date of Birth.

a. Instead of providing your Social Security Number, we have povided your MICA Employee ID number to M&T Bank.  The number must be nine digits, wo the ID is padded with zeros. If your ID is 123456, then your "Social Security Number" would be 000123456.

b. Your Date of Birth that is provided to M&T Bank is the one on file with HR.
