Health History and Vaccinations
Please remember to complete your Health History Form (which was sent in a separate mailing) including documentation of a physical with your physician. The initial date for returning your health history form was June 6, 2012. If you have not turned in your completed health form with vaccination information, including meningitis vaccination, please know there is still time; the Office of Student Affairs has extended the deadline for health forms to July 20, 2012. Please mail health forms to the Office of Student Affairs, 1300 West Mount Royal Avenue, Baltimore, MD 21217.
You should have received a separate mailing containing detailed information about the State of Maryland's requirement that all students living in College housing receive the meningitis vaccination. If you have any questions about the vaccination requirements, student health services, or the Health History Form, please call the Office of Student Affairs at 410-225-2422.
Health Insurance
MICA has adopted the following policy that applies to our students; all students are required to maintain health insurance coverage that is in place every day of their higher education career.
All students attending MICA must enroll in or waive out of the insurance coverage offered by the College. Students who wish to waive out of the coverage must provide proof of comparable private coverage. At this time, a charge of $1,294.00, which includes all administration fees, has been placed on your MICA student account. Students are automatically enrolled into the MICA sponsored health plan, but they can choose to waive out of or confirm enrollment up to September 30, 2012. Any waivers requested after September 30, 2012 will be assessed a financial penalty. You will receive information in the mail about how to opt out of the insurance plan over the summer. Health Care Services
During the fall and spring semesters, student health services for MICA students are provided in the Wellness Center (410-225-4118), located at 1501 West Mount Royal Avenue. Every MICA student pays a mandatory health fee in order to access student health services. As a reminder, all enrolled students are encouraged to contact MICA Student Health Services before visiting the emergency room.
